More about our Organization

Organization serves:
primary population would be older adults who are sixty and older, however, over the past 4-5 years, our population has expanded and in our case management programs, particularly the waiver programs that use Medicaid dollars to keep people in their homes for as long as possible, we have clients that are younger than sixty.
In 2014 we entered into a contract with Care Source and provide case management in the Ohio Home Care Waiver (OHCW). The OHCW serves children as well: birth to 59. In our managed care programs, MyCare Aetna and MyCare Molina we serve people across the age spectrum.
In our information and assistance programs, we also are addressing needs that come to us from people across the age spectrum. We work with family caregivers, this may include grandchildren as well as adult children caring for a family member. When we’re not the best ones to provide the information needed we direct and link them to other areas such as the DD community.
